This year's theme is"The Next 100 Years: Reaching new heights in health protection,"which acknowledges and recognizes that health is influenced by many factors outside the health care system.The commitment and leadership in activities that address the broad determinants of health has never been stronger. Within the Canadian spectrum of health care,EPHPs are an integral and important component. Public health advances such as safer and healthier foods, control of infectious diseases,healthier environments and the recognition of tobacco use as a health hazard have contributed significantly to Canadians living longer. EPHPs are primarily employed by federal,provincial and local governments, First Nation Tribal Councils and also within the private sector. We are involved in the inspection, enforcement and education of a wide range of programs and services,including but not limited to: food premises, recreational water,healthcare and childcare facilities,personal service settings, drinking water systems,land use,sewage systems,air quality,radon,schools and the implementation of disease and health hazard controls. This letter of request is to initialize October 15th of every year to be "Arif Merani Day" in Niagara Falls. Arif was born, raised, and completed his education in Niagara falls, until he attended Ottawa's Carleton University. Arif was extremely active at the MacBain Center YMCA playing any number of sports after school and on weekends including playing for the Niagara Falls Crushers throughout high school. In Ottawa, he took a shine to giving back to the community as a volunteer working to organize vigils for abused women and children on a yearly basis. He was also deeply involved with the university's Ismaili Student's Association holding executive positions throughout his many years at Carleton in addition to being a liaison with the parent organization of Ottawa. Moreover, he become a Brother of the Kappa Sigma fraternity at Carleton that instilled in him the values of professionalism, honor, ideals of brotherhood, and most importantly—service to others. With all this, though, he was preparing to bring his newly honed skills back to Niagara where he eventually wanted to establish a career and start a new chapter in his life. He expressed much angst throughout his time at Carleton about missing Niagara and excitement when discussion his move back after his studies were completed. In Niagara — he was known by virtually everyone. Everyone knew Arif personally or knew someone that knew him personally. He kept people together, organized sporting events, and took part in city-wide activities with his peers. At the age of 22, Arif tragically passed while on route to returning to Ottawa's Carleton University after spending the 2013 Thanksgiving Holiday with his parents in Niagara Falls. Arif's undergraduate degree was intentionally selected with a legal career in mind. During his studies he had the opportunity to learn more about the varying social strata within Canada, and the entrenched nature of this dynamic between people and state. It was Arif's finding that the most favourable strategy to assist those who seemed unable to escape their own circumstance was to become a lawyer. He was especially inspired by the struggles experienced by abused women and the conditions of the aboriginal people. Since the passing of our son, all the acts of charity and kindness cannot fit within the confines of this letter, however, to highlight but a few to date — our Foundation has raised almost $140,000.00 to date. We have collected approximately $114,000 for the Aga Khan Foundation Canada to alleviate poverty and over$22,000 for Gillian's Place. These endeavours are ongoing and will not stop. We will continue to honour Arif's memory by continuing to find ways to provide for others in need. We wish to divulge this message of love, giving, generosity, charity, and service to others by having this day named after him to promote others to act in a similar fashion and to give back to others. Please let us know how we can hallmark this amazing life in our community who was taken prematurely. As another example of how the Merani family has been so active in our beloved Niagara Region —Arif's older brother— Dr. Aleem Merani —was an active member on the Mayor's Youth Advisory Committee (MYAC) throughout his high school years. He organized the subcommittee with Ms. Lori Albanese of the City of Niagara Falls and Mr. Chris Russell of Niagara Transit to find ways to improve ridership in Niagara. With YOUR help as well, Mr. Diodati - Aleem chaired the subcommittee to create, organize, plan, and execute the inception of the "Quarter-a-Ride" promotion that continued for many years and proved to be wildly successful. We are attaching the relevant sections of the original agreement for your reference. In accordance with Section I 7, we are attaching a copy of the Region's proof of insurance for your records. Please provide proof of your insurance as set out in Section 18. We are attaching a copy of the Region's Certificate of Insurance, and would suggest that this form should be forwarded to your broker, along with a copy of the relevant sections of the agreement, so it can be properly completed by your Insurer or Broker. Please inform your broker that this fellable form is also available on the Region's website — www.niagararegion.ca/business/fpr/cert-insurance.aspx. The form to be completed is the Standard Insurance form. The Region is required to be added as an additional insured. Please make sure this is checked in Box 4 of the certificate. CATHARINES TRANSIT COMMISSION, THE CORPORATION OF THE CITY OF NIAGARA FALLS AND THE CORPORATION OF THE CITY OF WELLAND (Hereinafter collectively called the "Service Providers") WHEREAS by an Agreement made effective as of 00:01 on the 24th day of September, 2010 (hereinafter called the "Original Agreement"), the Region and the Service Providers agreed that the Service Providers shall provide Services for a pilot project regarding transit services (hereinafter called the "Pilot Project"); AND WHEREAS the Original Agreement had a termination date of 23:59 hours September 5, 2014; AND WHEREAS the parties hereto desire to extend the Original Agreement to a revised termination date of 23:59 hours September 5, 2015 with all other terms to remain the same save as amended by this Agreement; NOW THEREFORE this in consideration of the sum of TWO DOLLARS ($2.00) and the exchange of other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the parties agree as follows: I.
